I do not participate as a provider with most health insurance companies.
However,
your sessions may still be covered by your health insurance through your "out of network" provider benefits.
Please check your coverage carefully by asking the following questions:

Do I have mental health benefits?

What is my deductible and has it been met?

How many sessions per calendar year does my plan cover?

How much does my plan cover for an out-of-network provider?

What is the coverage amount per therapy session?

Is approval required from my primary care physician?
Fee for a 50 minute therapy session is $100.
Payment is requested at time of service.
Most insurance plans provide an "out-of -network" benefit if you choose to see a provider who is not on your insurance panel. Check this out with your insurance plan.
I will provide you with a statement that will assist you in making a claim to your insurance company for reimbursement of fees paid.

I am a provider with the UNI plan. Call your EAP for a referral at (801)587-9319. Or call me first and I can assist in obtaining a referral on your behalf. UNI benefits typically allows 10-20 sessions per year. Co-pays per session $20.
